<span style="font-size: medium;" class="mycode_size"><span style="color: #8b8b8b;" class="mycode_color">{Note: that even when the ATSB decide not to investigate further they are still required to notify ICAO of this fact} With further digging I was able to get a brief snapshot from the ICAO incident/accident database which in most cases showed no record for the many ATC notified LOSA/BOS incidents that didn’t escalate to a full blown investigation/prelim &amp; final report. -</span></span></blockquote>
